ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
1. Implements physical therapy services in accordance with the plan of treatment developed by the physical therapist and signed by the physician and planned, delegated and supervised by the physical therapist.

2. Observes, records and reports the client’s reaction to treatment and any change in the client’s condition to the physician, case manager and/or physical therapist and other staff involved at least every 60 days.

3. Collaborates with the physical therapist in assisting the physician in assessing the client’s functional level. Assists in the development and revision of the plan of care.

4. Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care.

5. Instructs and advises clients, family and other health team personnel, in certain phases of physical therapy in which they may assist the client; instructs all of the above in the use of prosthetic and orthotic devices and durable medical equipment, if appropriate.

6. Assists in preparing client clinical notes and progress reports according to agency policy and time frames.

7. Attends client care conferences. Initiates client care conferences for complex and/or multidisciplinary clients when needed or helpful to ensure good coordination of care.

8. Develops, prepares and maintains individualized client care progress records with accuracy, timeliness and according to policies. Submits accurate documentation according to Agency guidelines.

9. Keeps abreast of therapy trends and knowledge.

10. Participates in inservice programs.

11. Participates in the agency’s quality improvement program.

12. Contributes to team effort by accomplishing related results as needed.

EDUCATION/EXPERIENCE:
Graduate of a two-year, college-level program approved by the American Physical Therapy Association or has two years of appropriate experience as a physical therapy assistant and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service. (Determinations of proficiency do not apply to persons initially licensed by a state or seeking initial qualification as a physical therapy assistant after December 31, 1977.)

CERTIFICATES, LICENSES, REGISTRATIONS - Valid physical therapist assistant license in all states which are part of catchment area. Must maintain patient care standards (i.e., CPR certification, OSHA compliance, TB testing, etc.). Valid drivers license.
